Sound bath with your companion animal
I acknowledge that I am taking part in a sound bath experience with my companion animal named above, alongside other animals and people, organised by Shanti Sounds and Shanti & Me LLC at the location and on the date noted above.
I understand that there are risks associated with this activity, including injury to myself, to my companion animal, and to other animals, and damage to personal property. Animals behave unpredictably, and there is a risk of injury if another animal becomes aggressive or disruptive.
My animal is well behaved in a group setting, is current on Rabies, Bordetella, and Distemper vaccination, and stays with me for the whole session. One animal per person. I will remove my animal from the room if they become distressed or disruptive.
I release and hold harmless Shanti Sounds, Shanti & Me LLC, and their officers, employees, agents, volunteers, and contractors from any and all claims, demands, suits, causes of action, or liability for any injury, loss, damage, or death arising from my participation in this activity, whether caused by the negligence of Shanti Sounds, Shanti & Me LLC, myself, my companion animal, or any other participant.
I acknowledge that I have read this release of liability and assumption of risk agreement, fully understand its terms, and voluntarily accept the risks associated with participating in this activity with my companion animal.
