Welcome to Shanti's Pack. Please complete every section and sign at the end. This agreement is between Shanti & Me LLC and the client named below.
I certify that my dog is in good health and able to take part in car rides, trail hikes, and outdoor activity, and is current on Rabies, Bordetella, and Distemper, with tags firmly affixed. My dog is on regular parasite prevention. I will tell Shanti & Me before any hike if my dog is ill, injured, or not well enough to attend.
I certify that my dog has not harmed or threatened a person or another dog, including in a vehicle, on a trail, or around toys, sticks, food, or other resources. My dog has basic training and reasonable recall, is comfortable riding in a vehicle, and is safe around other dogs in close quarters. I will disclose any reactivity or aggression before service, and any change in behavior as it appears.
Outdoor hiking carries inherent risk: terrain, wildlife, weather, water, exposure to bacteria or illness, and interaction with other dogs. Shanti & Me LLC uses safety protocols and reasonable care but cannot remove that risk.
I am solely responsible for any injury, damage, or harm caused by my dog while in the care of Shanti & Me LLC, and I release and hold harmless Shanti & Me LLC, its owner, employees, and agents, from any claim, damage, or expense arising from my dog's participation in hiking, walking, transportation, or related activity, except in cases of gross negligence.
Dogs may return dirty, muddy, wet, or with burrs, scratches, or minor cuts. That is the natural result of a real hike. Please check your dog over after each outing.
If my dog is injured or becomes ill, Shanti & Me will attempt to reach me and then my emergency contact. If neither can be reached, I authorise transport to the nearest appropriate veterinary facility, and I accept full financial responsibility for all treatment.
I give permission for Shanti & Me LLC to work with my dog and to encourage positive behavior while in care. I acknowledge that the handler and service provider is Shanti & Me LLC and its owner, and that Shanti & Me LLC is licensed and insured.
By signing below I acknowledge that I have read, understand, and agree to the terms of this agreement. Where signed electronically, my typed name is my legal signature under the Utah Uniform Electronic Transactions Act.
